The Challenge

Design a service that helps patients with certain long term conditions to change their behaviour and become more physically active in their everyday lives.

Help these patients achieve positive clinical outcomes.   

The Objectives

    Make it easier for patients and health care providers to track physical activity levels

    Give all patients the ability to use the service, regardless of their digital literacy

The Insight

Give health professionals an aggregated view of patient data.

To be able to fully support their patients, health professionals needed full remote access to patient activity levels.

With this access, they could directly support their patients' digital active lifestyles — syncing data and setting new targets.

The Process


Develop proof of concept — part of Storm ID's work on digital therapeutics


Tailor user-centred design processes to design an exercise referral scheme programme — to be piloted in NHS Forth Valley.


Identify key performance indicators for a three month pilot programme. 


User data and interviews used for a formal evaluation of the pilot by an independent team.
